Having a concrete time frame in mind means that your planning must include today as the start of the journey to your goal. A 5-year plan must be documented to the finest detail and must include activities you wish to perform on defined dates as well as a defined time frame for said activities.
There must be documented allowance periods for when time frames are exceeded or honored so you can pre-empt or accommodate any unforeseen situations.

For example: If your goal 5 years from now is to graduate with distinction at school, outline all the subjects you need to pass to get to that distinction and make a personal timetable that is tailored to that timeline.

In the example given above, a milestone can be in the form of a mock exam for the student to assess their performances in all the subjects they wish to pass.
